Documentation Guidelines
for Physical, Medical, or Mental Health disabilities

  • Students with physical, medical, or mental health disabilities are asked to provide documentation by an appropriate and qualified professional.


  • A diagnosis and the degree of severity of the condition by medical doctors, licensed clinical social workers (LCSW) psychologists, psychiatrists or neurologists are required.


  • If the student is on any medication, the name and dosage of the medication should be included in the documentation submitted.
Mental Health Concerns

It is recommended that any student with mental health concerns meet with Lynn Abarno and a counselor from the Wellness Center during the month prior to admission.

Handicapped Parking Permits

To obtain a permit to use designated disabled parking spaces you must submit a completed application to your village, town or city clerk's office. Applications can be obtained from DisAbility Support Services, Cobleskill Village Clerk (518-234-3891) or your hometown clerk's office.
